When a Dental Implant Does Not Fuse with Bone

A dental implant needs to become stable within the jawbone before it can support its final restoration. When this biological process, called osseointegration, does not occur as expected, the implant may remain mobile or develop other symptoms. The cause and next step must be assessed by a dentist or oral surgeon.

Possible signs of a problem

  • Movement of the implant or attached restoration
  • Persistent or worsening pain
  • Swelling, redness, discharge or an unpleasant taste
  • Difficulty biting or a change in the way the teeth meet
  • Changes seen during a clinical or radiographic review

Some discomfort and swelling can be expected after surgery, but symptoms vary. Contact the treating clinical team promptly if symptoms worsen or if you are concerned. Severe swelling, breathing difficulty, uncontrolled bleeding or signs of a serious infection require urgent medical attention.

Why integration may not occur

Possible contributing factors include infection, limited bone volume or quality, excessive movement or loading during healing, implant position, smoking, gum disease and some health conditions or medicines. A problem is not always caused by one factor, and it cannot be diagnosed from symptoms alone.

How it is assessed

The clinician may review the treatment history, examine the implant and surrounding tissues, assess mobility and bite, and request appropriate imaging. The aim is to determine whether the implant, restoration, gum or nearby structures are involved and whether infection or bone loss is present.

What may happen next

Management depends on the findings. It may include monitoring, cleaning or treating surrounding tissues, adjusting the restoration, removing the implant, allowing the site to heal, bone grafting, or considering another implant or a different tooth-replacement option. Retreatment is not automatically suitable, and no method can guarantee integration.

Healing and the timing of any further procedure differ between patients. Follow the treating clinician’s instructions about medicines, hygiene, diet, smoking, activity and review appointments rather than relying on a fixed online timetable.
