Age-Related Macular Degeneration: Symptoms and Treatment
Age-related macular degeneration (AMD) affects the macula, the central part of the retina used for reading, recognising faces and detailed vision. It usually affects central rather than peripheral vision. Dry and neovascular (“wet”) AMD behave differently and require different management.

What the Macula Does
The macula contains densely organised retinal cells needed for fine central detail and colour vision. AMD involves complex changes in the retinal pigment epithelium, photoreceptors, Bruch’s membrane and choroidal circulation; it is not simply caused by thinning or broken blood vessels.
Peripheral vision is often retained even in advanced AMD, so the condition does not usually cause complete darkness. Nevertheless, central loss can greatly affect reading, driving, face recognition and independent tasks.
Dry AMD and Geographic Atrophy
Dry AMD can include drusen and pigment changes and may progress slowly or remain stable for a time. Advanced dry AMD can produce geographic atrophy, in which areas of retinal support tissue and photoreceptors are lost.
Drusen alone do not determine prognosis. Their size and distribution, pigment changes, imaging findings, involvement of both eyes and other features help an ophthalmologist estimate risk and monitoring needs.
Neovascular or Wet AMD
Wet AMD develops when abnormal vessels grow beneath or within the retina and leak fluid or blood. It can cause distortion, central blur or a dark/missing area over a shorter period than typical dry AMD.
New straight-line distortion or central change requires prompt macular assessment. Similar symptoms can arise from other retinal conditions, so an Amsler grid or online image cannot establish the diagnosis.
Symptoms and Urgent Warning Signs
Symptoms can include difficulty reading, needing brighter light, reduced contrast, colours appearing less vivid, straight lines appearing bent, faces looking distorted or a central blank area. One eye can compensate for the other, delaying awareness.
Seek urgent eye care for sudden or new central distortion, a new central shadow, sudden loss of vision, flashes with a sudden increase in floaters, or a curtain across vision. Do not wait for travel or a routine remote review.
Risk Factors and Prevention Claims
Age, smoking, family history and genetic factors are important associations. Cardiovascular health, diet and other factors may also be relevant. Stopping smoking benefits eye and general health and is one of the most important modifiable steps.
A balanced diet, physical activity, blood-pressure care and appropriate sun protection support general health, but they do not guarantee prevention of AMD. Routine genetic tests do not currently replace clinical staging or determine treatment for most people.
Examination and Imaging
Assessment can include visual acuity, dilated retinal examination and optical coherence tomography. Fundus photography, autofluorescence, OCT angiography, fluorescein angiography or indocyanine-green angiography may be used depending on the suspected type and treatment decision.
Home monitoring with an Amsler grid or digital tool may help a diagnosed patient notice change, but it does not replace scheduled imaging or urgent assessment of new symptoms.
Treatment for Wet AMD
Intravitreal anti-VEGF medicines are commonly used to suppress abnormal vessel leakage and growth. Treatment schedules differ by medicine, response and eye. Repeated injections and OCT monitoring are often required; stopping or extending treatment is a clinical decision.
Risks include discomfort, bleeding on the eye surface, pressure change, inflammation, retinal tear or detachment and infection inside the eye. Severe pain, increasing redness or sudden worsening after an injection needs urgent care. Photodynamic therapy or other treatment has selected indications.
AREDS2 Supplements
AREDS2-formula supplements can reduce the risk of progression for selected people with intermediate AMD or particular advanced findings. They do not cure AMD, recover lost vision or prevent AMD in people without the relevant stage.
The exact formulation matters. High-dose supplements can interact with medicines and may be unsuitable with some health conditions. Former or current smokers should avoid formulations containing beta-carotene because of lung-cancer risk. An ophthalmologist or prescriber should confirm indication and product.
Dry AMD Devices and Emerging Treatment
Photobiomodulation devices, complement-inhibitor injections and other treatments have specific regulatory status, eligibility, evidence and risks that vary by country. A device session schedule or promotional visual-acuity result should not be copied as a treatment recommendation.
For geographic atrophy, some approved treatments aim to slow lesion growth rather than restore vision and may carry injection-related or retinal risks. Stem-cell and gene therapies remain investigational for many proposed uses. Ask whether a treatment is licensed locally and what patient-relevant benefit was demonstrated.
Low-Vision Rehabilitation
Low-vision assessment can help with magnification, lighting, contrast, electronic readers, accessibility software, eccentric-viewing strategies and daily-living adaptations. Rehabilitation is useful alongside medical treatment rather than only after all treatment has ended.
Driving eligibility depends on measured vision and local standards. Visual hallucinations can occur with significant sight loss (Charles Bonnet syndrome); they should be discussed with a clinician rather than hidden through fear of stigma.
