Cosmetic Ear Surgery: Assessment, Techniques and Risks

Cosmetic ear surgery, also called otoplasty or pinnaplasty, changes selected ear cartilage, position or shape. It cannot create idealised symmetry or guarantee confidence, psychological change or lifelong satisfaction.

Assessment and consent

The surgeon should assess ear anatomy, asymmetry, skin, cartilage, hearing concerns, health and goals. For a child or young person, their own wishes and ability to participate in aftercare are important. Alternatives include no surgery.

Procedure

Techniques may reshape, score, fold or remove selected cartilage through incisions commonly placed behind the ear. The NHS England ENT policy describes otoplasty as cartilage remodelling to create folds or position the ear closer to the head, but the individual plan varies.

Risks and recovery

Risks include bleeding or haematoma, infection, pain, altered sensation, asymmetry, recurrence, cartilage irregularity, visible or raised scars, skin problems and revision. Headbands, sleep position, sport, work or school follow the individual instructions. An anonymous satisfaction percentage or “science and art” quotation does not predict outcome.
