Frenuloplasty in Turkey from South Africa: a plain guide
Frenuloplasty is a short operation for a tight or short frenulum (frenulum breve), the small band of tissue on the underside of the head of the penis that joins it to the foreskin. When that band is too short it can pull, tear or bleed, and make erections or intercourse painful. This page explains what the procedure involves, how it differs from circumcision, and what South African patients should plan for before travelling to İzmir.
In short: Frenuloplasty lengthens or releases the frenulum while keeping the foreskin. It is usually done under local anaesthetic as an outpatient procedure and takes around 20 to 30 minutes. Most men are back to light daily activity within one or two days, avoid sexual activity while it heals, and are fully healed in four to six weeks. A urological examination comes first, and the doctor decides when you are fit to fly home.

What frenuloplasty is, and what it is not
The aim is to relieve tension in the frenulum, not to remove it. The surgeon makes a small cut in the tight band and reshapes it so that it becomes longer and more flexible, then closes it with fine dissolvable stitches that do not need to be removed. The result is functional rather than cosmetic: the goal is comfort, not a change in appearance.
It is not the same as circumcision. The foreskin stays in place, which is why some men choose frenuloplasty as a tissue-preserving option. In selected cases a doctor may suggest circumcision instead; see our circumcision guide if that has been raised with you.
Who it suits
Frenuloplasty is considered when symptoms are clearly linked to a short or tight frenulum, for example:
- A pulling sensation or pain during erections
- The head of the penis bending downwards when erect
- Repeated tearing or bleeding of the frenulum
- Discomfort during intercourse
- Men who want to keep their foreskin
Active infection or inflammation, a bleeding disorder, uncontrolled diabetes or penile pain without a clear cause all need careful assessment first. A medical history and physical examination come before any decision, and the doctor will also discuss alternatives such as conservative management in mild cases.
The procedure and the first weeks
Local anaesthetic is used in most cases; general anaesthesia is rarely needed. After the procedure, mild swelling, bruising, soreness and temporary sensitivity are common and usually improve within a few days.
- Keep the area clean and dry, and follow the hygiene instructions you are given
- Wear loose underwear
- Avoid sexual activity and masturbation until the doctor says healing is complete
- Most men return to non-strenuous daily activity within 24 to 48 hours
- The stitches dissolve on their own, and full healing usually takes four to six weeks
Planning the trip from South Africa
There are direct flights from Johannesburg and Cape Town to Istanbul of around ten hours, followed by a short domestic flight to İzmir. South Africans need an e-Visa before travelling, and İzmir is only one hour ahead of South Africa.
Because this is a short procedure under local anaesthetic, most men need only a few days in İzmir, but allow time for a check before you leave rather than flying out the same day. On the flight, wear loose clothing and keep the area clean. The doctor decides when you are fit to fly.
Before you book, tell your GP at home that you are planning the procedure, and take your discharge notes back with you. South African medical schemes generally do not cover elective treatment abroad, so check with your scheme, and note that standard travel insurance usually excludes planned treatment abroad. Risks
Frenuloplasty is considered low risk, but infection, bleeding, scarring, temporary changes in sensitivity, incomplete relief of symptoms and, rarely, the need for a second procedure are all possible.
