Gastric Sleeve vs Bypass: Differences, Risks and Follow-up
Gastric sleeve and gastric bypass are different metabolic and bariatric operations. Neither is automatically the better choice: the decision depends on health, eating patterns, reflux, medicines, previous surgery and the individual’s ability to follow long-term nutritional care.
How the procedures differ
A sleeve gastrectomy removes a large part of the stomach to create a narrower stomach. A gastric bypass creates a small stomach pouch and reroutes part of the small intestine. Bypass changes absorption more substantially, while sleeve does not reroute the intestine. Both require permanent changes to eating and follow-up.
Assessment and alternatives
A specialist team should review body-mass index in its clinical context, obesity-related conditions, reflux, diabetes, nutrition, psychological readiness, smoking, medicines and previous attempts at weight management. Alternatives may include structured non-surgical care, medication or another operation where clinically appropriate.
Risks and long-term care
Both operations can involve bleeding, infection, blood clots, leaks, anaesthetic complications, gallstones, nutritional deficiencies, inadequate weight change or weight regain and further procedures. Bypass also carries risks related to internal hernia, marginal ulcer and dumping symptoms; sleeve can worsen reflux in some patients. Individual risk cannot be represented by an unattributed percentage.
Patients need an agreed plan for diet progression, supplements, blood tests and clinical review. Weight change, remission of associated conditions and recovery vary and are not guaranteed.
