Identify the cause before choosing treatment

Under-eye appearance may be influenced by fat position, skin, facial structure, pigmentation, fluid retention, allergies or other health factors. The consultation should include eye and eyelid history, dry-eye symptoms, medicines, previous procedures, health conditions and smoking or nicotine.

Vision complaints require appropriate eye assessment. Cosmetic lower-eyelid surgery should not be presented as a general treatment for peripheral-vision loss.

Surgical approaches and alternatives

A lower-eyelid incision may be placed inside the eyelid or through the skin near the lashes, depending on the required changes. Fat may be removed or repositioned and skin treatment may be discussed. Fillers, skin treatments and surgery have different indications and risks; none is automatically best.

Risks

Risks can include bleeding, infection, dry or irritated eyes, scarring, asymmetry, altered sensation, eyelid-position change, difficulty closing the eyes, persistent swelling, vision disturbance, dissatisfaction and revision. Sudden or worsening vision problems after surgery require urgent medical assessment.

Recovery and aftercare

Bruising, swelling, watering, temporary blurred vision from ointment and discomfort may occur. Drops, wound care, cooling, sleep position, work, driving, screen use, exercise, flying and contact lenses should follow the treating team’s instructions. Fixed recovery dates and invisible-scar promises are not reliable.

Results and limitations
