Ab-interno Surgeries for Glaucoma in Infants

Ab-interno glaucoma surgery in infants and children approaches the eye’s drainage angle from inside the front chamber. Goniotomy and related angle procedures may be considered for selected childhood glaucomas, but they are specialist operations and are not interchangeable with adult “minimally invasive glaucoma surgery”.

Why childhood glaucoma is different

Raised eye pressure during eye development can enlarge or cloud the eye and damage the optic nerve. Causes include primary congenital glaucoma and glaucoma associated with another eye or systemic condition. Examination under anaesthesia is sometimes needed to measure pressure and assess the cornea, angle, optic nerve and eye size.

Goniotomy and other options

Goniotomy opens selected drainage tissue using a view through a sufficiently clear cornea. Trabeculotomy approaches the drainage channel differently. When angle surgery is unsuitable or insufficient, a specialist may consider filtration surgery, a drainage implant or cyclodestructive treatment. The sequence depends on diagnosis, anatomy, previous procedures and response.

Risks and long-term care

Risks include bleeding, inflammation, infection, corneal damage, cataract, pressure that remains too high or becomes too low, scarring and further surgery. Published percentages from mixed diagnoses cannot predict an individual child’s result. Children often require repeated examinations, vision correction and amblyopia treatment as well as pressure control.
