Rotator cuff repair in Turkey from South Africa: a plain guide

The rotator cuff is a group of four muscles and their tendons that hold the top of the arm bone in the shoulder socket and let you lift, rotate and reach. A tendon can tear in a fall, through heavy lifting or repeated overhead work, or simply wear with age. Rotator cuff repair reattaches the torn tendon to the bone. The operation is the easy part; for South African patients the planning is about flying home in a sling and months of physiotherapy after you return.

In short: Rotator cuff repair is done by board-certified orthopaedic surgeons in İzmir, usually by keyhole surgery under general anaesthesia. Small, recent tears repair far more predictably than large, long-standing ones. Expect a sling for the first weeks and four to six months or longer of rehabilitation, most of it at home.

Who it suits

  • People with a large or complete tendon tear
  • People with shoulder pain that continues, often worse at night
  • People with clear weakness when lifting or rotating the arm
  • People whose symptoms limit daily life despite physiotherapy or medication

Some smaller tears improve with rest, physiotherapy and medication. The surgeon needs your MRI or ultrasound scan, any X-rays and a clear history, including when the problem started. How large the tear is, how long it has been there and the quality of the tendon decide what a repair can realistically achieve.

How it is done

  • Arthroscopic repair — through small incisions with a camera; the tendon is reattached to the bone with stitches or small anchors
  • Mini-open repair — keyhole surgery combined with a small incision, for larger or more complex tears
  • Open repair — a larger incision, for severe tears or where reconstruction is needed

The operation is done under general anaesthesia and takes one to two hours. More detail is on the rotator cuff repair treatment page.

How long to stay, and the flight home

After the operation the arm is held in a sling or immobiliser to protect the repair, and you cannot use it to lift or carry. The surgeon decides when you are fit to fly; ask for the planned length of stay in your written treatment plan and book a return ticket you can change.

  • Travel with a companion if you can; you will not be able to lift luggage
  • Ask your airline for assistance at the airports
  • Ask the surgeon how to manage pain and the sling on a ten-hour flight, and about clot prevention
  • Bring your scans, operation notes and the rehabilitation plan home with you

Rehabilitation at home

Rehabilitation moves through stages: keeping the shoulder still while the tendon heals, then gentle movement guided by a physiotherapist, then active strengthening, and finally a return to normal use. Light activities usually return within a few weeks, but full recovery takes four to six months or longer. Driving and work depend on which arm was operated on and what you do; ask the surgeon.

Book a physiotherapist near you before you travel and share the surgeon’s protocol. Follow-up with the surgical team continues by message and video call; Turkey is only one hour ahead of South Africa. Medical complications arising from the treatment, and any revision that becomes clinically necessary as a result, are covered for one year from the date of treatment by our treatment guarantee. Any such treatment is carried out in İzmir by the treating team; flights and accommodation remain your own responsibility.

Risks

Possible complications include infection, bleeding, shoulder stiffness, nerve injury, slow healing and the tendon tearing again. Following the sling and physiotherapy instructions reduces these risks. Other sports and overuse injuries are covered in our sports injuries guide and the wider orthopaedic surgery guide for South African patients.

What we will not do

We will not confirm a plan before a surgeon has reviewed your scans and history, we will not promise a result, and we will not operate if your health makes it unsafe. A long journey is not a reason to operate when conservative treatment has not been tried, or to do more in one operation than the surgeon judges safe.

Frequently asked questions

Can a rotator cuff tear heal without surgery?

Some smaller tears improve with rest, physiotherapy and medication. Larger or complete tears with ongoing weakness and pain may need surgical repair.

How long does rotator cuff repair take?

Usually one to two hours under general anaesthesia, depending on the size of the tear and the technique used.

How long after rotator cuff repair can I fly home to South Africa?

The surgeon decides. You will be in a sling and unable to lift luggage, so travel with a companion if you can and book a return ticket you can change.

How long does recovery take after rotator cuff repair?

Light activities usually return within a few weeks, but full recovery takes four to six months or longer, with physiotherapy throughout.

Will my medical aid cover rotator cuff repair in Turkey?

South African medical schemes generally do not cover planned treatment abroad, and standard travel insurance usually excludes it. Ask both specifically before you book.
