Forehead reduction in Turkey from South Africa: a plain guide
Forehead reduction, also called hairline lowering, shortens a tall forehead by moving the hairline down. It is a small operation in terms of time away, but it leaves a permanent scar along the front of the hairline, so it is worth understanding exactly what it does and does not do before you plan a trip from South Africa to İzmir.
In short: The surgeon removes a strip of forehead skin just in front of the hairline and brings the hair-bearing scalp forward. The change in forehead height is permanent. It does not lift the eyebrows or smooth forehead lines. Most patients are cleared to fly after five to seven days, and South Africans need an e-Visa and a connection from Istanbul to İzmir.

What the operation does
A band of skin is taken out along the front edge of the hairline, and the scalp behind it is advanced to close the gap. The incision follows the hairline so that the scar sits where hair can soften its appearance. The aim is a forehead that is better in proportion with the rest of the face, while your expressions stay your own.
It is a different operation from a brow lift. A brow lift raises drooping eyebrows; forehead reduction shortens the forehead and leaves the brows where they are. If your main concern is low or heavy brows, a brow lift or an endoscopic brow lift may suit you better. The technique is described in more detail on the forehead reduction treatment page.
Who it suits
- People with a naturally high hairline or a long forehead who want a permanent change
- People whose scalp has enough looseness (laxity) to be moved forward; the surgeon checks this
- People who have been told they are not suitable for lowering the hairline with a hair transplant
- People who accept a scar along the hairline
Send clear photographs of your forehead and hairline from the front and side. The surgeon decides from these, and from the examination in İzmir, whether the operation is suitable. Forehead reduction can be combined with other facial procedures such as brow, eyelid or nose surgery, but only where the surgeon judges the combination safe.
How long to stay, and the flight home
Direct flights from Johannesburg or Cape Town to Istanbul take around ten hours, followed by a short domestic flight to İzmir. You will need an e-Visa before you travel. On arrival you have health checks, blood tests and a face-to-face meeting with the surgeon before the operation.
Most patients are cleared to fly home after five to seven days, once the surgeon has checked the wound. Do not book the shortest possible trip: choose a return ticket you can change, in case the surgeon wants to see you again before a long-haul flight.
- Expect some swelling, tightness and numbness of the forehead and front of the scalp on the flight
- Keep the incision clean and dry as instructed, and avoid pressure on it from hats or headrests
- Walk the aisle regularly and drink water on the long flight
- Take your discharge notes and medication list home with you
Recovery at home
Most patients describe mild discomfort, swelling, tightness and temporary numbness rather than strong pain, and these are managed with prescribed medication. Most people return to desk-based work and light daily activities within one to two weeks. Strenuous exercise waits until the surgeon clears you. The scar is usually most visible in the first months and becomes much less noticeable over time.
Follow-up continues by photograph, message and video call with the same team; İzmir is only one hour ahead of South Africa. Tell your GP at home about the operation. Medical complications, and any revision that becomes clinically necessary, within one year of treatment are covered by our treatment guarantee; it does not cover a preference for a different look where the result is within what was discussed before surgery. For other facial options, see the face surgery guide for South African patients.
What we will not do
We will not confirm a plan before a surgeon has reviewed you, and we will not promise a particular hairline. We will not offer forehead reduction to someone whose real concern is brow position or forehead lines, and we will not add procedures to one operation because the journey is long.
Frequently asked questions
How long do I need to stay in Turkey for forehead reduction?
Most patients stay five to seven days, which allows for the operation, follow-up checks and the surgeon’s approval before the long flight home to South Africa. Book a return ticket you can change.
Will I have a visible scar?
Yes, there is a scar along the hairline. It is placed so that it heals as discreetly as possible, and it usually becomes much less noticeable over time.
Is forehead reduction the same as a brow lift?
No. Forehead reduction lowers the hairline to shorten the forehead, while a brow lift raises drooping eyebrows. The surgeon recommends one or the other based on your anatomy.
Are the results permanent?
The lower hairline is permanent, although the face continues to age naturally over time.
Will my medical aid cover forehead reduction in Turkey?
Probably not. South African medical schemes generally do not cover elective treatment abroad, and standard travel insurance usually excludes planned treatment abroad. Ask your scheme and insurer directly before you book.
