DHI hair transplant in Turkey from South Africa: a plain guide

Direct Hair Implantation (DHI) is a hair transplant in which each follicle is placed with an implanter, giving the clinician close control over the angle, direction and depth of every graft. It is often chosen for rebuilding a hairline or adding density. Like any transplant, it moves your own hair: it cannot create new donor hair or stop hair loss that is still progressing. This page complements our South African hair transplant guide.

In short: DHI may suit men and women with pattern hair loss who have enough donor hair. The procedure usually takes around five to six hours, and most patients are cleared to fly home after two to three days, subject to the surgeon’s approval. New growth usually starts from around three to four months, and the result keeps developing for up to 18 months.

How DHI works

On arrival you have health checks, blood tests and a face-to-face meeting with the surgeon. Follicles are then taken from the donor area and implanted one by one into the thinning area. The implanter lets the clinician set the angle, direction and depth of each graft, which matters most at the hairline, where hair has to sit naturally. The procedure usually takes around five to six hours.

The choice between DHI and FUE is the clinician’s, based on your hair and donor area; our FUE page for South African patients explains the other technique.

Who DHI may suit

  • Men and women with thinning, a receding hairline or pattern hair loss
  • People with enough healthy donor hair
  • People wanting a more precise hairline or more density in a defined area
  • Non-smokers, or people able to stop before surgery
  • People with realistic expectations of the result and the recovery

Suitability is confirmed only after a medical review of your hair loss pattern, donor area, medical history and photographs. If another treatment would suit you better, you should be told. Women should also read our female hair transplant page, and patients with curly or Afro-textured hair the hair-type section of our South African hair transplant guide.

Travel and timings from South Africa

From Johannesburg or Cape Town, a direct flight to Istanbul takes around ten hours, followed by a short domestic flight to İzmir, where treatment takes place. South Africans need an e-Visa before travelling; our e-Visa guide explains how to apply.

The two-to-three-day figure is the surgeon’s decision on the day, not a booking rule, so book a return ticket you can change, and keep anything from rubbing the grafts on the ten-hour flight.

South African medical schemes generally do not cover cosmetic hair procedures abroad, and standard travel insurance usually excludes planned treatment abroad. Check both before you book.

What recovery looks like

  • First three days: mild redness, swelling and small scabs around the grafts; avoid touching or washing the area unless the surgeon says otherwise
  • First week: most scabs fall away and light daily activity usually resumes
  • Weeks two to eight: the transplanted hairs usually shed; this is expected, while the follicles remain beneath the skin
  • From three to four months: new growth begins, with clear improvement for most patients by nine to twelve months and the result still developing at 18 months

Protect the scalp from strong South African sun and ask when a hat and scalp sunscreen are allowed. Turkey is one hour ahead, so follow-up by photograph and message is easy.

Risks

DHI is surgery. Possible problems include swelling, redness, temporary numbness, scabbing, itching, minor bleeding, infection, folliculitis and uneven growth. Some grafts may not survive, leaving lower density than hoped.

Our treatment guarantee covers medical complications and clinically necessary revisions for one year from the treatment date; that treatment is done in İzmir at no charge, but flights and accommodation are your own cost. It does not cover a preference for a different look within what was discussed. Our complaints page explains how problems are handled.

What we will not do

We will not promise a density or a hairline, take more grafts than your donor area can safely give, or proceed if a transplant is not the right answer to the cause of your hair loss. Our main DHI hair transplant page has further detail.

Frequently asked questions

How long do I need to stay in Turkey for a DHI hair transplant?

Most patients are cleared to fly home after two to three days, subject to the surgeon’s approval. Allow for health checks and a surgeon meeting on arrival, and book a return ticket you can change.

How long does a DHI procedure take?

Usually around five to six hours, depending on your plan.

Is DHI suitable for women?

It can be. DHI is used for men and women with pattern hair loss, but hair loss in women has many causes, so a medical review comes first.

When will I see results after DHI?

The transplanted hairs usually shed in the first weeks. New growth begins from around three to four months, most patients see clear improvement by nine to twelve months, and the result can keep developing for up to 18 months. Some grafts may not survive.

Will my medical aid cover DHI in Turkey?

South African medical schemes generally do not cover cosmetic hair transplants, and standard travel insurance usually excludes planned treatment abroad. Ask both before you book.
